Leaving a hospital setting can be overwhelming, especially without the right support. Our Peer Specialists are here to walk alongside individuals as they transition back into the community, helping them stay connected to care, resources, and people who understand.
Step-down support that meets people where they are
Our team works with partners across San Antonio to support individuals as they move from institutional care into everyday life. Whether it’s connecting them to ongoing programs or helping them navigate their next steps, our Peer Specialists provide compassionate, experience-driven support tailored to each person’s needs.
Connect to a Community that Cares
We help individuals transition smoothly into the San Antonio Clubhouse—a free, welcoming community that supports adults with a history of mental illness. Open weekdays from 8 AM to 4 PM, the Clubhouse offers opportunities to volunteer, learn, build friendships, and regain purpose. Our Peer Specialists guide individuals through the onboarding process and follow up to ensure continued engagement.
Supporting Reintegration After Long-Term Care
In partnership with the Center for Healthcare Services, we support patients exiting the Texas State Hospital system as they move into shared housing in San Antonio. Our Peer Specialists provide in-home support and community reintegration services, helping individuals build routines and find connection as they settle into their new lives.
Custom Step-Down Support for Your Hospital
We work with private hospitals to design customized Peer Support services that ease the transition from inpatient care to independent living. Whether it’s follow-up visits, coordinated handoffs, or emotional support after discharge, our flexible model is built around your process and your patients’ needs.
